What changed

One Alt-text: line was authored per record under the pass-3 B-track register constraint (deadpan, concrete visual inventory, evidence-bounded; exemplar mascots/M-0019, pilot PR #993). None of the 29 assigned records carried an existing ## Image section; one was created per record at the conventional position:

Before ## Biography where that heading exists, including the emoji-headed variant on 403 (## 🧠 Biography).
Before the equivalent first headed narrative section where no ## Biography exists (319 → ## Archival Witness; 322, 323 → ## Classification; 324 → ## Failure Domain; 325 → ## Overview; 326, 327 → ## Failure Signature; 418 → ## Limericks).
On verse-only records (313–318, 320, 321, 400, 405) the unlabeled lead prose functions as biography and was left intact; ## Image was placed after it, before ## Aphorisms.
On 320, floating ### Witness Function/### Distinction H3s precede the verse tail; ## Image was placed before ## Aphorisms so the H3s remain lead matter rather than visually nesting under the new H2.
On 324, 326, and 327, which separate headed sections with --- dividers, ## Image was placed after the first --- and before the first headed section, so the divider retains its lead-vs-sections role.

What was deliberately left alone

All frontmatter, canonical IDs, titles, tags, statuses, and relations.
No other wording, ordering, verse tails, or formatting. No frontmatter image: keys or .assets/ references were introduced; the Alt-text: line is body text only.
Non-formatting defects noticed while reading (e.g. 409's filename/title drift ledger-snag vs Ledger Snarl, 405's mismatched limerick heading Method Not Allowed — Rest Request Verb Rejection, 320's floating H3s, 400's ## Archival Records inside the limerick tail) were left as-is; residue is not this slice's business.
